Our entire hospital district is committed to providing every patient and their families in the Ferriday, Sicily Island, Jonesville, and surrounding Northeast Louisiana communities with quality, progressive, compassionate healthcare, regardless of their race, belief system, sex, or ability to pay for care. Our Mission & Vision

At the end of the day, our focus is not on profits. It’s on our people. We strive to surpass barriers to affordable, accessible healthcare for families in the Catahoula and Concordia Parish areas by providing insurance enrollment support, sliding fee programs, and comprehensive care options available all in one local healthcare system. With this approach, our patients can receive community-driven care from local experts in primary care, dental services, behavioral and mental health, diagnostic testing, and immunizations. 

OUR HISTORY

Healthcare Rooted in
Our Community

The vision for a local health center began in 1975 with members of the Sicily Island Rotary Club who realized our community was facing a critical shortage of healthcare professionals and growing health disparities. To assist with this, the Police Jury created a hospital district and passed a local tax initiative to help fund what has become Catahoula Parish Hospital District #2.

Construction began in 1978 and was completed in 1979. For the first time in 25 years, the district successfully recruited both a physician and a dentist. By 1980, full-time medical and dental services were available to the community.

In 1994, grant funding made it possible to build our current 10,000-square-foot facility in Sicily Island. Under the leadership of former CEO Mrs. Emma Tarver, services expanded to include clinics in Wisner, Ferriday, and Jonesville, along with the addition of a mobile dental unit to reach even more patients.

Today, with ten providers serving our region, we remain committed to the same mission to serve the community that shaped us from the beginning.

Chief Executive Officer,
Debra Miesch

Debra Miesch is the Chief Executive Officer of Catahoula Parish Hospital District #2 (CPHD2), an organization that has served communities since 1979, expanding across Catahoula, Concordia, and Franklin Parishes.

Under her leadership, CPHD2 has undergone extensive modernization and service expansion, upholding an unwavering commitment to patient care and regulatory excellence.

Ms. Miesch has overseen the renovation and opening of new facilities, including a dental clinic, a mental health facility, and mammography services. She has helped established an up-to-date computer system, X-Ray equipment, laboratory equipment, a modern billing department, created new Human Resource and Infection Control Departments, hired qualified staff, and promoted hardworking employees. Ms. Miesch’s dedication ensures CPHD2 continues to meet high standards set by organizations like The Joint Commission, HRSA, CLIA, and COLA, assuring communities of the best possible care.

Roger Smith

Medical Doctor

Dr. Smith, originally from Jonesville, LA, attended Louisiana Tech University, LSU School of Medicine, and completed his residency at the University of Kentucky Hospital. He is board certified in both pediatrics and internal medicine. He has chosen to come back to his roots in order to serve those in his surrounding communities. Dr. Smith serves as our medical director.

Huey H. Moak

Medical Doctor

Dr. Moak is a long time practicing physician for the Ferriday area. He has owned and operated his own practice for 36 years. He is a member of Louisiana State Medical Society.

Susan Howard

Nurse Practitioner

Susan Howard-Crawford, originally from Mangham, LA, attended NLU and Grambling State University. She is affiliated with the American Association of Nurse Practitioners as well as, the Louisiana Association of Nurse Practitioners. Susan has chosen to serve her community and devote her career to personalized care for her patients at Island Medical.

Paige Frey

Nurse Practitioner

​Paige Frey, born and raised in Monterey, LA. She and her husband have been married for 4 years and have a beautiful daughter. She graduated from Monterey High School in 2014. She then attended ULM where she obtained her BSN in nursing in 2018. She graduated from NSU in May 2024 with MSN. She has 5+ years of experience in emergency room and surgery. She is a member of the American Academy of Nurse Practitioners. She is certified with Basic Life Support, Advanced Cardiovascular Life Support, Pediatric Advanced Life Support, Trauma Nurse Core Course, and Emergency Nursing Pediatric Course. Paige is excited to be able to serve and give back to the people of this area who have played a crucial role in accomplishing her dream as a Nurse Practitioner. You can visit Paige at Ferriday Family Health and Ferriday Health West (Dr. Moak’s office).

Ruby Iva Ward

Nurse Practitioner

Iva is a Psyche Nurse Practitioner who is  no stranger to Patients in the MIssLou. She has served in a local hospital for many years and continues to offer her patients expert behavioral care through our Ferriday and Jonesville Clinics. Ever ready to serve, Iva would love to hear from you  if you are in need of  her services.  

Sarah Duncan

Nurse Practitioner

Sarah Duncan is a graduate of Alcorn State University and McNeese State University. She is a board-certified Psychiatric Mental Health Nurse Practitioner dedicated to providing compassionate, evidence-based mental health care to individuals across the lifespan. With a strong foundation in both nursing and psychiatric care, Sarah specializes in the diagnosis, treatment, and management of a wide range of mental health conditions while promoting recovery and empowering patients to achieve lasting wellness through medication management and therapeutic interventions.

Samantha Strange

Nurse Practitioner

Samantha Strange has been married for 18 years with two kids.  She graduated from Block High School in 2002.  LSUA-ASN-2005, Northwestern-BSN-2020, & University of South Alabama-MSN-2022.  She has 17 years in Pediatrics & Pediatric Intensive Care, 1 year in Labor & Delivery.  She has a professional organization membership with American Academy of Nurse Practitioners.  She is also certified with Basic Life Support, Advanced Cardiac Life Support, Neonatal Resuscitation Program & Emergency Nursing Pediatric Course.  Being entrusted with the care of others is her highest honor.  Healthcare providers have a duty to provide compassionate, timely, ethical care to all patient population.  She chose to be a Family Nurse Practitioner because she wants to make a positive impact in health care.  She considers herself Blessed to be able to care for others through all stages of their lives.  "To know even one life has breathed easier because you have lived: That is to have succeeded." Jacob Crawford

Nurse Practitioner

Jacob Crawford, originally from Winnsboro, La., attended Southeastern Louisiana University for his BSN and MSN degrees. Jacob is affiliated with the American Association of Nurse Practitioners. He currently lives in Harrisonburg , La. with his wife, Sydni. You can visit Jacob in our Jonesville Medical Clinic , where he enjoys caring for people across the lifespan.

Brenda Brown

Nurse Practitioner

Brenda Brown, FNP, is a dedicated Family Nurse Practitioner with more than 26 years of diverse clinical experience providing compassionate, patient-centered care across multiple healthcare settings. Her extensive background includes primary care, pediatrics, geriatrics, women’s health, cardiology, neurology, long-term care, and preventive health planning. Brenda is committed to helping patients achieve their best possible health through personalized treatment plans, preventive care, and education tailored to each individual’s needs. Her broad clinical expertise and compassionate approach allow her to care for patients across all stages of life while building lasting relationships founded on trust and respect. Known for her attentive listening and commitment to quality care, Brenda strives to create a welcoming environment where patients feel supported, informed, and empowered in their healthcare journey.

Susan “Libby” Morphis

Nurse Practitioner

Libby Morphis is dual certified in family and psychiatric mental health. She specializes in mental health issues across the lifespan and is certified in addiction treatment. She treats a variety of conditions, ranging from depression and anxiety to other serious mental illnesses, and provides medication-assisted treatment for individuals with alcohol and opioid use disorders.
